Taking the first step toward hospice can feel overwhelming. Our admissions team is here to walk with you through the process and answer your questions.
What Happens When You Call
When you reach out, a member of our admissions team will:

Listen to your situation and talk with you about how hospice can help

Ask for some basic information

Contact your primary physician or specialist (such as an oncologist or cardiologist) if you are being treated for a serious illness
To help us get started, we’ll ask for:

Patient information: name, date of birth, home address, and where the patient is currently staying

Primary caregiver or healthcare proxy: name and contact information

Physician information: name and contact details for your primary doctor or specialist
Our admissions staff will then gather medical information and records from your doctor, including the diagnosis. Once we have what we need, we’ll call you to schedule an in-person visit with a member of our team.
What Comes Next
After your admission visit, our team will work with you and your family to create a plan of care that fits your needs and wishes.
