Launching in late winter, Hospicare will offer palliative care—an in-home medical service that provides expert care and emotional support for people living with a serious illness, even while they continue treatment.

Palliative care is for any age and any diagnosis. It helps when you need support with symptoms, medical decisions, or the stress that comes with serious illness.

You do not need a terminal diagnosis.

The goal of palliative care is simple: help you feel better and live as fully as possible.

Your care is based on your needs, goals, and values.

If you’re dealing with pain, shortness of breath, anxiety, nausea, fatigue, or stress, we help you find relief and feel more in control.

We also support your loved ones. Caregivers and family members often feel overwhelmed. Our team helps them, too.

Palliative care is specialized medical care for people living with a serious illness. It helps:

  • manage symptoms
  • reduce stress
  • provide emotional support

Unlike hospice, palliative care can be provided at any stage of illness and alongside treatment.

Visits happen where you live, including:

  • private homes
  • assisted living
  • skilled nursing facilities
  • other residential settings

We work with your doctors and specialists to coordinate care.

Most visits are covered by Medicare, Medicaid, and private insurance. Depending on your plan, you may have a co-pay.

We’ll help you understand your coverage and any possible out-of-pocket costs. No one is turned away because they can’t pay.

Your Palliative Care Team

When you sign up for palliative care services, you recieve in-home visits from providers who specialize in the care of individuals with serious illness. Your team will include:

  • Nurse Practitioner – helps manage symptoms, answers medical questions, and coordinates with your doctors 

  • Licensed Clinical Social Worker – provides emotional support, helps with planning, and connects you to resources

Visits are scheduled based on your needs, and we'll stay in touch with your medical providers to best coordinate your care.
