Hospicare’s palliative care services meet you where you are. We focus on what matters most to you and your family, and we provide care in the place you call home.
For Patients: Support where you are.
If you’re living with a serious illness—such as cancer, heart disease, COPD, or another life-changing condition—palliative care brings relief and guidance to you.
Here’s what care often looks like:
Nurse Practitioner (NP) visits every 6–12 weeks to:
- manage symptoms like pain, nausea, fatigue, or shortness of breath
- answer questions about your condition and treatment options
- coordinate with your other doctors so your care matches your goals
Licensed Clinical Social Worker (LCSW) visits every 8–12 weeks to:
- provide emotional support
- talk through future plans and decisions
- connect you with helpful services and community resources
Between visits, our team is available to:
- answer questions
- consult with your medical team
- adjust your care plan if your needs change
You stay in charge. You keep your own doctors. You can continue treatment if you choose. Our role is to add support—so you can live as well as possible.
For Families: Guidance you can trust
Being a caregiver can feel heavy. You shouldn’t have to carry it alone. As a family member or caregiver, you can expect:
- clear communication about what’s happening and what to expect
- emotional support, especially during hard conversations
- help navigating healthcare systems, insurance questions, and care planning
- peace of mind knowing you have a team beside you

Palliative care is a relationship. We work with you, your loved ones, and your current medical providers. We check in regularly, help with decisions, and adjust as your needs change.
Some people use palliative care for a few months. Others stay with us for years. There’s no time limit—only support for as long as it helps.
