From the moment of diagnosis through the final stages of illness and beyond, it’s natural to feel overwhelmed, stressed, and unsure of what’s next.

At Hospicare, we believe caregiving should never feel like a burden you have to carry alone. Our full team is here to guide you, care for your loved one, and support you throughout this journey.

The Hospicare Care Team: Here for You and Your Loved One

When you choose Hospicare, you’re not just getting one provider—you’re gaining an entire team of compassionate experts who work together to support you and your family. Your care team may include:

  • Primary Nurse Case Manager: A registered nurse specializing in hospice and palliative care, managing your loved one's physical needs in consultation with your doctor.
  • Social Worker: Offers emotional support and practical help, like navigating insurance, legal documents, and funeral arrangements.
  • Home Health Aide or Licensed Practical Nurse: Assists with personal care and daily activities to help your loved one stay comfortable.
  • Spiritual Care Coordinator: Provides counseling that respects your spiritual beliefs and offers support around grief and meaning at end-of-life.
  • On-Call Nurse: Available 24/7 to answer urgent questions or visit your home if needed. Simply call (607-272-0212).
  • Volunteers: Offer companionship for your loved one and provide you with breaks through respite care.
  • Bereavement Counselor: Supports you and your family for up to 13 months after your loss through individual counseling, groups, and mailings.
  • Medical Director: Consults with your primary physician to ensure you receive the best, most coordinated care possible.
  • Primary Physician: Your regular doctor stays involved, with ongoing updates and collaboration from our team.

Every patient is assigned a core team: a primary nurse, social worker, and bereavement counselor. Other specialists are added based on your loved one’s needs and your family's wishes

Together, we create a personalized care plan that can include:

  • Managing pain and symptoms
  • Providing emotional, psychosocial, and spiritual support
  • Supplying medical equipment (hospital beds, shower chairs, and more)
  • Teaching caregiving skills like medication management, wound care, and in-bed mobility
  • Preparing you for changes as your loved one’s condition evolves
  • Offering grief support before and after your loved one’s passing

Caring for Yourself as a Caregiver

Your well-being matters, too. Caring for someone else can be all-consuming, but it’s important to take care of yourself so you can continue to be there for your loved one.

  • Eat regular, balanced meals.
  • Exercise, even if it’s just a short walk outside.
  • Get quality sleep whenever possible. Ask someone you trust to stay with your loved one so you can rest.
  • Ask for help—and accept help when it’s offered.
  • Make a list of tasks others can assist with, like grocery shopping, cooking, dog walking, or lawn care.
  • Share caregiving responsibilities with family, friends, or volunteers.
  • Take time for yourself. You don’t need to give up the activities you love. Even a short break can recharge you.
