Getting the Right Support Shouldn't Be Complicated

Whether you’re a patient, a family member, or a healthcare provider, reaching out is the first step. Anyone can call. You do not need a doctor’s referral to start the conversation.

We’re here to answer questions, explain how palliative care works, and help you decide if it’s a good fit.

What to Expect

After your call:

  • An admissions specialist will ask a few basic questions.

  • If the call comes from a patient or family member, we’ll connect with the patient’s primary care provider to review medical history and coordinate care.

  • We’ll schedule the first visit—typically within 10 days.

During the first visit, we will:

  • explain our palliative care services and how we work with your current doctors

  • talk with you and your family about your goals and concerns

  • review symptoms and current needs

  • help you start any paperwork

We recommend calling early. Palliative care works best when we have time to build a relationship with you and your care team.

If you’re a patient or family member

If you or a loved one is living with a serious illness and needs more support—symptoms, treatment decisions, or emotional stress—palliative care may help.

We can support you:

  • at home
  • in a nursing facility
  • in other residential settings

If you’re a physician or healthcare provider

We work alongside referring providers to support patients through:

  • symptom management
  • care coordination
  • emotional support

You do not need to complete a full workup before referring. We’ll coordinate with you to gather the medical information we need.
